Chronic Problem Management

Chronic Problem Management involves establishing an ongoing relationship with patients who contend with long-term illnesses or conditions, such as to manage active medications, recurring checkups, and following test results and trends.

JonkokeMed™ provides you, as physician, with easy-to-use tools for tracking a patient's chronic care needs, and for comparing this to current clinical practice guidelines. You can then provide the patient with information about how their condition compares to the guidelines, and what they need to plan for with regard to future appointments.

The Encounter screen provides visual cues about a patient's special needs or conditions. These graphics, called Stickies, provide one-click access to historical data related to that condition, which can be reviewed or graphed.
